C4 Night Camp Waiver and Release of Liability

C4 MOBILE GAMING – LIABILITY WAIVER & RELEASE OF CLAIMS (MINOR)

This Liability Waiver & Release of Claims (“Agreement”) is entered into by and between C4 Mobile Gaming LLC (“Company”) and the undersigned parent/legal guardian (“Guardian”) on behalf of the minor participant (“Participant”).

2. Acknowledgment of Risk

I understand that participation in C4 Night Camp activities, including but not limited to:

Video gaming
Virtual reality (VR)
Physical movement within and around the gaming truck
Outdoor/indoor recreational activities

involves inherent risks, including but not limited to:

Slips, trips, and falls
Motion sickness or disorientation (VR)
Minor or serious physical injury

I voluntarily assume ALL risks, known and unknown, associated with my child’s participation.

3. Release of Liability

To the fullest extent permitted by law, I hereby release, waive, and discharge:

C4 Mobile Gaming LLC, its owners, officers, employees, contractors, affiliates, and agents

from any and all liability, claims, demands, or causes of action arising out of or related to:

Injury, illness, or death
Property damage or loss
Participation in camp activities

This release applies even if caused by negligence of the Company, except in cases of gross negligence or willful misconduct where prohibited by law.

4. Medical Authorization

In the event of an emergency, I authorize C4 Mobile Gaming staff to:

Seek emergency medical treatment for my child
Contact emergency services if necessary

I accept full financial responsibility for any medical care provided.

5. Health Disclosure

I certify that:

My child is physically able to participate
Any medical conditions, allergies, or behavioral concerns have been disclosed in advance

Failure to disclose relevant information releases the Company from any related liability.

6. Behavior & Removal Policy

I understand that:

Participants must follow all rules and staff instructions
Unsafe, disruptive, or inappropriate behavior may result in immediate removal without refund

7. Photo & Media Release

I grant permission for my child to be:

Photographed
Videotaped

for use in:

Marketing materials
Social media
Website content

No compensation will be provided.

8. Personal Property

I understand that C4 Mobile Gaming is not responsible for:

Lost
Stolen
Damaged personal items

9. Indemnification

I agree to indemnify and hold harmless C4 Mobile Gaming from any claims, damages, or expenses (including legal fees) arising from:

My child’s participation
My breach of this Agreement

10. Governing Law

This Agreement shall be governed by the laws of the State of Georgia.

11. Severability

If any portion of this Agreement is found to be invalid, the remaining provisions shall continue in full force and effect.

12. Acknowledgment & Signature
